Electronic Online Documents Disclosure & Agreement

By continuing, you accept the terms of this Disclosure and Agreement, and hereby authorize United Bank to provide your bank account statements, notices, and tax forms and any related disclosures that we might send to you with these documents, electronically.

The same terms apply with respect to electronically delivered documents as for those delivered in paper form. The deposit agreement and disclosures that you previously received from United remain in effect. The type of online documents available are statements, notices, and tax forms. The types of accounts which are available to receive statements electronically are Checking, Savings, Money Market, Loans and Retirement. Not all statement, notices and tax forms may be available electronically.

Your Consent to Receive Electronic Delivery of Online Documents and Right/Option for Paper Delivery of Said Documents.  Documents that you would normally receive in written form, you are consenting to receive electronically and you can retain in an electronic form. In order to provide you with electronic documents you will provide us with an email address that will be used to send you all related notifications. You will let us know immediately if your email address changes. You may change your email address at any time in our Online Banking System. You will receive an email notification when you have a new electronic document available for viewing within our Online Banking System and you understand that it is your responsibility to view, print and/or save your statements for future reference.

You may also receive electronically, in addition to your online document, any materials that would have gone out with the online document including disclosures and promotional materials. At our election, we may deliver this electronic material to you by posting a notice and making the information available to you in our Online Banking System, or by sending the information to an email address you have provided to us.

Termination of Consent to Receive Online Document by Electronic Means. You may withdraw your consent to receive electronic online documents and resume receiving paper copies by changing your document delivery method. You can change your document delivery method for your accounts at any time within our Online Banking System; however, the change must be made at least ten days prior to your document date to ensure you receive a printed version for the current cycle; otherwise, you may not receive a paper document until the following cycle.

Requesting a Written Paper Copy of Account Statements Received Electronically. You may request a paper copy of any Account Statement, Online Document, or other information received electronically under this Agreement by notifying us. All requests must include the applicable account number(s) and specific period(s) being requested; refer to the current fee schedule pertaining fees involved. You can notify us by the following methods: (1) via email within our Online Banking System; (2) by visiting a United Bank location; or (3) by calling our Customer Service Center and speaking with an associate during our normal business hours.

System Access. Access to this service may be unavailable at times due to scheduled maintenance, unscheduled maintenance, or system outage. In addition, both environmental and physical events that occur may cause the system to become unavailable. United Bank will make every reasonable effort to ensure optimum availability of this system; however, we are in no way liable for the unavailability of the system or any damage that may result from system unavailability. United Bank disclaims any and all liability that relates to the improper use of this system. We are not responsible for any damage that may occur to your personal computer from the use of this service or the data transmitted. We reserve the right to deny you the ability to access, to limit access or to revoke your access to this service without advance notice.

System Requirements. In order to use the Online eBanking Service, you must obtain and maintain, at your expense, compatible hardware and software as specified by us from time to time. We are not responsible for any third-party software you may need to use the Online eBanking Service. Any such software is accepted by you as is and is subject to the terms and conditions of the software agreement you enter into directly with the third-party software provider at time of download and installation.

Your Responsibility for Maintaining the Security of your Password. Your User ID and Password are highly sensitive and extremely confidential and must not be disclosed to others or recorded in or on your personal computer. You agree not to disclose your User ID or Password to anyone. You understand that in providing this information to a third party, you are granting that party the right to view your online documents, which will include but not be limited to your account numbers, your account balances, your social security number/tax identification number, your account history and front and back images of your cleared checks. United Bank will accept no responsibility for any resulting losses you incur.
